I have taken Policy No: D0610012704 Dtd.26/12/2009 yearly amount of Rs.10350/- matured on 26/06/2015 at Durg district in chhattisgarh . But your Durg branch office was closed since last few years ago. I am enclosing here with the policy certificate copy and payment receipt for your ready reference. So, request you to kindly refund my amount at my correspondence address which was mentioned in my policy.
