Subject: : HDFC ERGO – Reimbursement Claims Query Request-RR-HS17-10471309

Dear Sir,

The Following reply have been submitted by around five times (which has been certified by the concerned Doctor who made my treatment) to the above company against their repeatedly same query but the company asking same query again and again despite releasing my health insurance claim. The last reply from my end dated 12/12/2017 emailed to the company (Email: [email protected]) is reproduced below for your kind information and take up appropriate action against them:

Hospital Name : RAMA MEDICAL COLLEGE,HOSPITAL and RESEARCH CENTRE
Hospitalization Period : From 04/09/2017 to 09/09/2017

I am again shocked to receive your same query on several times reply to which have already been submitted to you day by day by way of the treating doctor’s certificate. I am quoting your query below which speaks for itself.

Quote:

“Kindly provide exact duration of DM TYPE -2 with Old glioma in brain since when first time detected certify from treating doctors .,”

Unquote:

The reply to this query have already been submitted by me many times duly certified by the Concerned Doctor quoted below which is self explanatory.

Quote:
Doctor’s Certificate against the above query:

“So on 5/9/17 he is diagnosed with T2DM”. (This has been certified by the concerned Doctor on a certificate which has already been submitted to the above company on 31/10/2017 under their official receipt).

Unquote:

This is as per Doctor’s diagnostic report. I do not know what do you want any more information above that this clarification. I once again submit a receipted copy of the Doctor’s Certificate (as the original has already been submitted to your office on 31.10.2017 under cover of your official receipt.

May I now request you to release my health insurance claim at the earliest without any further delay.
